Discharge Instructions
<p><strong>Diagnosis:</strong></p>

<li>Vertigo</li>
<li>Hypertension</li>
</ul>
<p><strong>Medications:</strong></p>
<ul>
<li><strong>Meclizine:</strong> Take 25 mg by mouth 3 times a day (TID) as needed for dizziness.</li>
</ul>
<p><strong>Care Instructions:</strong></p>
<ul>
<li>Move slowly when changing positions to help reduce dizziness.</li>
<li>Avoid driving or operating heavy machinery if you feel dizzy.</li>
<li>Monitor your blood pressure at home, if possible, and record the results to discuss with your doctor.</li>
</ul>
<p><strong>Follow-Up:</strong></p>
<ul>
<li>Follow up with your Primary Care Physician (PCP) this week to discuss your blood pressure management and address any ongoing symptoms.</li>
</ul>
<p><strong>Reasons to Return to the Emergency Department:</strong></p>
<ul>
<li>New or worsening neurological symptoms (such as weakness, numbness, difficulty speaking, or severe headache)</li>
<li>Persistent or worsening dizziness despite medication</li>
<li>Chest pain, shortness of breath, or sudden vision changes</li>
</ul>
